Here are two components to help support safe reduction or cessation of opioid use:

1. Example Taper‑Down Plan (for prescribed opioids)

Based on BC and Canadian guidelines, a safe taper is:

  1. Reduce by 5–10 % of your current daily dose every 2–4 weeks

  2. If you're more anxious or were on high doses long-term, slow it further—5 % every 1–3 months

  3. Monitor symptoms closely—pain, mood, withdrawal

  4. Pause or slow the taper if withdrawal becomes overwhelming or pain spikes

  5. Add symptom relief as needed—non-opioid pain meds (NSAIDs), OTC clonidine, anti-nausea, Imodium, etc. reddit.com+15www2.gov.bc.ca+15helpstartshere.gov.bc.ca+15

  6. Maintain regular check-ins (weekly to monthly) with a healthcare provider or support person

This taper can typically take 2 weeks to 4 months, or longer if coming off high doses .

Never abruptly stop opioids, especially after long-term use—this can cause severe withdrawal, potential overdose after tolerance drops, or even suicidal thoughts reddit.com+9www2.gov.bc.ca+9camh.ca+9.

2. Local Supports in Vancouver & BC

To support your taper and recovery journey, here are accessible and credible resources:

Summary: Safe & Supported Path Forward

  • Use a gradual taper: 5–10 % dose reduction every 2–4 weeks, slower if needed

  • Combine with medical and emotional support: through OAT, clinics, counseling, peer groups

  • Never taper alone: withdrawal symptoms and risks mean supervision is vital

  • Stay connected: to professionals, peers, and loved ones—community reduces relapse risk
